ABSTRACT

As Infecções Relacionadas à Assistência à Saúde (IRAS) ocorrem com mais frequência em Unidade de Terapia Intensiva (UTI) devido a exposição maior dos pacientes a procedimentos e dispositivos invasivos, quadro clínico debilitado e sua manipulação pela equipe assistencial exigindo uso elevado de antimicrobianos, o que pode promover um risco de desenvolvimento de resistência bacteriana a estes, cujas consequências podem ser a dificuldade de tratamento, internamento prolongado, risco de óbito e maior custo associado. Tem como objetivo descrever as IRAs relacionando os agentes etiológicos e o tratamento antimicrobiano em uma UTI de um hospital de referência da mesorregião do Rio Grande do Norte. Trata-se de um estudo descritivo, retrospectivo e transversal de abordagem quantitativa. Foram inseridos 1.682 pacientes internados na UTI geral do hospital estudado entre 2017-2020. Os dados foram coletados a partir de fichas de registro que foram tabuladas e analisadas nos softwares Microsoft Office Excel® e Statistical Package for the Social Sciences utilizando estatística descritiva simples com apresentação de frequências, tendências e dispersão. A análise dos resultados revelou mediana de idade de 57 anos, prevalência do sexo masculino e existência de comorbidades em 57,9% dos casos, especialmente infecção prévia a admissão na UTI. O tempo médio de permanência na UTI foi 11,4 dias e taxa de mortalidade de 52%. Quanto aos dispositivos invasivos, observou-se uso de sonda vesical de demora (96,8%), ventilação mecânica (79,4%) e cateter venoso central (83,7%). Constatou-se 790 IRAS da UTI com crescimento bacteriano em 48,2%. As principais densidades de incidência (DI) de IRAS/1.000 pacientes-dia foram: IPCSL-CVC 1,8; PAV 27 e ITU-AC 22,3. Quanto aos antibióticos, observou-se Lenght of therapy de 872,5/1.000 pacientes-dia, sendo os mais prescritos: vancomicina (N=633), meropenem (N=625), ceftriaxona (N=479), amicacina (N=463) e polimixina B (N=448). Os valores destaques de Days of therapy/1.000 pacientes-dia: meropenem (N=305,7), amicacina (N=260,4), polimixina B (N=256,4), vancomicina (N=229,3) e imipenem (N=165,3). As bactérias mais isoladas nas culturas foram: Acinetobacter spp., Pseudomonas spp. e Klebsiella spp., as quais apresentaram resistência, principalmente, a: ceftazidima (51,5% - 87,3%); cefepima (61,6% - 85,3%); ciprofloxacino (56% - 84,6%) e meropenem (31,7% - 80,3%). Identificou-se não conformidades no tratamento com antibióticos em 455 pacientes, que envolvem principalmente polimixina B, vancomicina, meropenem e ceftriaxona. Conclui-se que há elevados níveis de tempo de permanência na UTI e uso de dispositivos invasivos, assim como DI de IRAS alta com identificação microbiológica de bactérias importantes, especialmente por seu perfil de resistência acentuado com destaque para antibióticos da classe dos carbapenêmicos e cefalosporinas de 3a e 4a geração. Destaca-se também a presença de não conformidades na administração de antibióticos que podem contribuir para a seleção de bactérias multirresistentes.


Health Care-Related Infections (HAI) occur more frequently in the Intensive Care Unit (ICU) due to the greater exposure of patients to invasive procedures and devices, weakened clinical status and their handling by the care team, requiring high use of antimicrobials, which can promote a risk of developing bacterial resistance to these, whose consequences may be difficult treatment, prolonged hospitalization, risk of death and higher associated costs. It aims to describe the IRAS relating the etiological agents and antimicrobial treatment in an ICU of a reference hospital in the mesoregion of Rio Grande do Norte. This is a descriptive, retrospective and cross-sectional study with a quantitative approach. A total of 1,682 patients admitted to the general ICU of the hospital studied between 2017-2020 were included. Data were collected from registration forms that were tabulated and analyzed in Microsoft Office Excel® and Statistical Package for the Social Sciences software using simple descriptive statistics with presentation of frequencies, trends and dispersion. The analysis of the results revealed a median age of 57 years, prevalence of males and the existence of comorbidities in 57.9% of cases, especially infection prior to admission to the ICU. The average length of stay in the ICU was 11.4 days and the mortality rate was 52%. As for invasive devices, the use of an indwelling urinary catheter (96.8%), mechanical ventilation (79.4%) and central venous catheter (83.7%) was observed. There were 790 IRAS in the ICU with bacterial growth in 21.67%. The main HAI incidence densities (DI)/1,000 patient-days were: IPCSL-CVC 1.8; PAV 27 and UTI-AC 22.3. As for antibiotics, a Length of therapy of 872.5/1,000 patient-days was observed, with the most prescribed being: vancomycin (N=633), meropenem (N=625), ceftriaxone (N=479), amikacin (N= 463) and polymyxin B (N=448). The highlighted values of Days of therapy/1000 patient-days: meropenem (N=305.7), amikacin (N=260.4), polymyxin B (N=256.4), vancomycin (N=229.3) and imipenem (N=165.3). The most isolated bacteria in cultures were: Acinetobacter spp., Pseudomonas spp. and Klebsiella spp., which showed resistance mainly to: Ceftazidime (51.5% - 87.3%); cefepime (61.6% - 85.3%); ciprofloxacin (56% - 84.6%) and meropenem (31.7% - 80.3%). Non-compliance was identified in the treatment with antibiotics in 455 patients, which mainly involve polymyxin B, vancomycin, meropenem and ceftriaxone. It is concluded that there are high levels of ICU length of stay and use of invasive devices, as well as high IRAS ID with microbiological identification of important bacteria, especially due to their accentuated resistance profile, with emphasis on antibiotics from the carbapenem class and cephalosporins from 3rd and 4th generation. Also noteworthy is the presence of non-compliance in the administration of antibiotics that may contribute to the selection of multidrug-resistant bacteria.


Las Infecciones Relacionadas con la Atención de la Salud (IRAS) ocurren con mayor frecuencia en la Unidad de Cuidados Intensivos (UCI) debido a la mayor exposición de los pacientes a procedimientos y dispositivos invasivos, el debilitamiento del estado clínico y su manejo por parte del equipo asistencial, requiriendo un alto uso de antimicrobianos , lo que puede promover un riesgo de desarrollar resistencia bacteriana a estos, cuyas consecuencias pueden ser un tratamiento difícil, hospitalización prolongada, riesgo de muerte y mayores costos asociados. Tiene como objetivo describir las IRAs que relacionan los agentes etiológicos y el tratamiento antimicrobiano en una UTI de un hospital de referencia en la mesorregión de Rio Grande do Norte. Se trata de un estudio descriptivo, retrospectivo y transversal con enfoque cuantitativo. Se incluyeron un total de 1.682 pacientes ingresados en la UCI general del hospital estudiado entre 2017-2020. Los datos fueron recolectados a partir de formularios de registro que fueron tabulados y analizados en el software Microsoft Office Excel® y Statistical Package for the Social Sciences utilizando estadística descriptiva simple con presentación de frecuencias, tendencias y dispersión. El análisis de los resultados reveló una mediana de edad de 57 años, predominio del sexo masculino y la existencia de comorbilidades en el 57,9% de los casos, especialmente infección previa al ingreso en UCI. La estancia media en la UCI fue de 11,4 días y la tasa de mortalidad fue del 52%. En cuanto a los dispositivos invasivos, se observó el uso de catéter urinario permanente (96,8%), ventilación mecánica (79,4%) y catéter venoso central (83,7%). Había 790 IRAS en la UCI con crecimiento bacteriano en el 48,2%. Las principales densidades de incidencia (DI) de IRAS/1.000 pacientes-día fueron: IPCSL-CVC 1,8; PAV 27 y UTI-AC 22.3. En cuanto a los antibióticos, se observó una Duración de la terapia de 872,5/1.000 días-paciente, siendo los más prescritos: vancomicina (N=633), meropenem (N=625), ceftriaxona (N=479), amikacina (N= 463) y polimixina B (N=448). Los valores destacados de Días de terapia/1.000 pacientes-día: meropenem (N=305,7), amikacina (N=260,4), polimixina B (N=256,4), vancomicina (N=229,3) e imipenem (N=165,3). Las bacterias más aisladas en cultivos fueron: Acinetobacter spp., Pseudomonas spp. y Klebsiella spp., que mostraron resistencia principalmente a: ceftazidima (51,5% - 87,3%); cefepima (61,6% - 85,3%); ciprofloxacino (56% - 84,6%) y meropenem (31,7% - 80,3%). Se identificó incumplimiento en el tratamiento con antibióticos en 455 pacientes, los cuales involucran principalmente polimixina B, vancomicina, meropenem y ceftriaxona. Se concluye que existen altos índices de estancia en UCI y uso de dispositivos invasivos, así como elevado IRAS ID con identificación microbiológica de bacterias importantes, especialmente por su acentuado perfil de resistencia, con énfasis en antibióticos de la clase carbapenémicos y cefalosporinas de 3ra y 4ta generación. También es destacable la presencia de incumplimiento en la administración de antibióticos que pueden contribuir a la selección de bacterias multirresistentes.

ABSTRACT

Abstract The prevalence of depression varies from 1 to 17% in different geographic regions, and its incidence is 70% higher in women than men. Today, depression affects more than 300 million people worldwide, affecting twice as many women from adolescence to adulthood. In addition to this earlier onset, depression in women tends to be more severe. Cardiovascular disease and depression are chronic diseases that have a major impact on cardiovascular and all-cause morbidity and mortality, with evidence of a two-way relationship between them, in which depression is a predictor of cardiovascular disease and vice versa. In females, the degree of illness and prognosis are more severe when both diseases are present, than when diagnosed alone. In patients with acute or chronic cardiovascular disease, especially women, a systematic screening for depression should be considered as a preventive strategy of cardiovascular events, aiming to reduce the risk of future events. There are still no clinical studies designed to assess the impact of antidepressant treatment on cardiovascular outcomes in women.

ABSTRACT

Chagas disease causes an important impact on cardiorespiratory system, functional and working capacity of the patient. Performing a functional assessment on these individuals becomes essential, since the impact of physical limitation is an important factor that is related with other life domains, such as risk of cardiovascular events in a brief period and risk of death. The goal of this work was to perform a literature review to present the evaluative and prognostic value of main valid respiratory and functional tests in patients with Chagas disease, as well as their applicability and reproducibility. A literature search was performed from 1981 to 2020 in Scielo, Pubmed/Medline, Science Direct, Scopus and Web of Science databases. Only complete studies published in English, Portuguese or Spanish were analyzed which specifically discussed the respiratory and functional tests used in patients with Chagas disease. In total, 544 articles were retrieved of which 35 were eligible. The main functional tests approached were Cardiopulmonary Exercise Testing, Incremental Exercise Testing and Six-Minute Walk Test. Several studies show themselves as useful markers for predicting quality of life, mortality and functional impairment, in addition to other patients' characteristics. Several tests can be used to assess the respiratory and functional impact to patients' health, as well as its relationship with important domains of their lives

ABSTRACT Background: Bleeding in hemophiliacs can cause complications in the central and peripheral nervous system (CNS and PNS). The incidence of intracranial hemorrhage has reduced after the introduction of prophylactic treatment with factor VIII or IX, but the benefits of this therapy have not yet been evaluated on PNS complications. Objective: The aim of this study was to determine the prevalence of neurological complications in hemophiliacs and verify the effect of prophylactic therapy in these patients, including PNS disorders. Methods: We retrospectively evaluated the prevalence of CNS and PNS disorders caused by bleeding in hemophiliacs seen at the Hemocentro Regional Norte, Ceará, Brazil, from 1992 to 2018, and we compared the incidence in different periods (before and after the introduction of prophylactic treatment in 2011). Results: Of 75 hemophilia A patients evaluated (4.61/100.000 population), 13.3% (n=10) had either CNS (n=5) or PNS (n=5) disorders secondary to bleeding. Patients submitted to factor VIII replacement prophylactic therapy were less likely to have CNS events: from 1992 to 2011, 5 of 63 patients had CNS disease, while from 2011 to 2018, there were no new cases (p=0.0181). From 2011 to 2018, 5 PNS events occurred in patients without prophylactic therapy, whereas none occurred in those covered by prophylactic therapy (5/20 versus 0/29, p=0.0081). Conclusions: The prevalence of neurological complications in hemophiliacs in our cohort is similar to other studies. Similar to CNS, prophylactic therapy also reduces the risk of PNS complications. This is the first report in the literature showing this benefit.


RESUMO Antecedentes: O sangramento em hemofílicos causa complicações no sistema nervoso central e periférico (SNC e SNP). A incidência de hemorragia intracraniana diminuiu após a introdução da profilaxia com fator VIII ou IX, entretanto esse benefício ainda não foi avaliado no SNP. Objetivo: O objetivo deste estudo foi determinar a prevalência de complicações neurológicas em hemofílicos, verificando o efeito da terapia profilática também no SNP. Métodos: Avaliamos retrospectivamente a prevalência de complicações neurológicas causadas ​​por sangramentos em hemofílicos atendidos no Hemocentro Regional Norte, Ceará, Brasil, de 1992 a 2018, comparando a incidência em diferentes períodos (antes e depois da introdução do tratamento profilático em 2011). Resultados: Foram avaliados 75 pacientes com hemofilia A (4,61/100 mil habitantes). Deles, 13,3% (n=10) tinham distúrbios do SNC (n=5) ou do SNP (n=5) secundários a hemorragias. Os pacientes submetidos à terapia profilática com fator VIII apresentaram menor probabilidade de eventos do SNC: de 1992 a 2011, cinco de 63 pacientes apresentaram hemorragia no SNC, enquanto de 2011 a 2018 não ocorreram novos casos (p=0,0181). De 2011 a 2018, cinco eventos no SNP ocorreram entre pacientes sem terapia profilática, e nenhum ocorreu entre aqueles cobertos pela profilaxia (5/20 × 0/29, p=0,0081). Conclusões: A prevalência de complicações neurológicas em hemofílicos em nossa coorte é similar à de outros estudos. Assim como no SNC, a terapia profilática também reduz o risco de complicações no SNP. Este é o primeiro relato na literatura a mostrar esse benefício.

ABSTRACT

Introdução: A pré-eclâmpsia com características graves (PECG) é uma síndrome específica da gravidez caracterizada por hipertensão grave e disfunção de órgãos-alvo e está associada a eventos cardiovasculares adversos a curto prazo, incluindo insuficiência cardíaca, edema pulmonar e acidente vascular cerebral. Objetivos: Os autores tiveram como objetivo caracterizar as alterações ecocardiográficas, clínicas e laboratoriais de curto prazo em mulheres com PEC, concentrando-se na pressão sistólica do ventrículo direito (PSVD) e nos parâmetros diastólico, sistólico e de rastreamento diastólico, sistólico e de salpicos derivados do ecocardiograma. Métodos: Neste estudo observacional prospectivo, os autores recrutaram 63 mulheres com PEC e 36 pacientes controle grávidas. Resultados: A coorte PEC apresentou maior PSVD (31,0 ± 7,9 mm Hg vs. 22,5 ± 6,1 mm Hg; p <0,001) e diminuição da tensão sistólica longitudinal global do VD (RVLSS) (19,6 ± 3,2% vs. 23,8 ± 2,9% [p < 0,0001]) quando comparado com a coorte controle. Para os parâmetros cardíacos do lado esquerdo, houve diferenças (p <0,001) na velocidade e 'septal mitral (9,6 ± 2,4 cm / s vs. 11,6 ± 1,9 cm / s), relação E / e' septal (10,8 ± 2,8 vs. 7,4 ± 1,6), tamanho da área atrial esquerda (20,1 ± 3,8 cm2 vs. 17,3 ± 2,9 cm2) e espessura da parede posterior e septal (mediana [intervalo interquartil]: 1,0 cm [0,9 a 1,1 cm] vs. 0,8 cm [0,7 a 0,9 cm] e 1,0 cm [0,8 a 1,2 cm] vs. 0,8 cm [0,7 a 0,9 cm]). Oito mulheres (12,7%) com PEC apresentaram disfunção diastólica grau II e 6 mulheres (9,5%) apresentaram edema pulmonar periparto. Conclusões: Mulheres com PEC apresentam maior PSVD, maiores taxas de função diastólica anormal, menor RVLSS global, maior remodelamento da câmara do lado esquerdo e maiores taxas de edema pulmonar periparto, quando comparadas a mulheres grávidas saudáveis.


Introduction: Pre-eclampsia with severe features (PEC) is a pregnancy-specific syndrome characterized by severe hypertension and end-organ dysfunction, and is associated with short-term adverse cardiovascular events, including heart failure, pulmonary edema, and stroke. Objectives: The authors aimed to characterize the short-term echocardiographic, clinical, and laboratory changes in women with PEC, focusing on right ventricular (RV) systolic pressure (RVSP) and echocardiographic-derived diastolic, systolic, and speckle tracking parameters. Methods: In this prospective observational study, the authors recruited 63 women with PEC and 36 pregnant control patients. Results: The PEC cohort had higher RVSP (31.0 ± 7.9 mm Hg vs. 22.5 ± 6.1 mm Hg; p < 0.001) and decreased global RV longitudinal systolic strain (RVLSS) (19.6 ± 3.2% vs. 23.8 ± 2.9% [p < 0.0001]) when compared with the control cohort. For left-sided cardiac parameters, there were differences (p < 0.001) in mitral septal e' velocity (9.6 ± 2.4 cm/s vs. 11.6 ± 1.9 cm/s), septal E/e' ratio (10.8 ± 2.8 vs. 7.4 ± 1.6), left atrial area size (20.1 ± 3.8 cm2 vs. 17.3 ± 2.9 cm2 ), and posterior and septal wall thickness (median [interquartile range]: 1.0 cm [0.9 to 1.1 cm] vs. 0.8 cm [0.7 to 0.9 cm], and 1.0 cm [0.8 to 1.2 cm] vs. 0.8 cm [0.7 to 0.9 cm]). Eight women (12.7%) with PEC had grade II diastolic dysfunction, and 6 women (9.5%) had peripartum pulmonary edema. Conclusions: Women with PEC have higher RVSP, higher rates of abnormal diastolic function, decreased global RVLSS, increased left-sided chamber remodeling, and higher rates of peripartum pulmonary edema, when compared with healthy pregnant women. ABSTRACT Objective To describe cervical cancer mortality rates and their corresponding trends, and to analyze the spatial correlations of this type of cancer in Natal-RN, Brazil, between 2000 and 2012. Materials and Methods The simple linear regression model, the empirical Bayes method and the Global Moran's index were used for the statistical analysis. Results The mortality coefficient of cervical cancer in Natal, standardized by age range, was 5.5 per 100 000 women. All historical series for the coefficients studied were classified as stable. The Global Moran's index obtained was 0.048, with a p-value for the spatial test correlation between neighborhoods of 0.300. The average family income by neighborhood showed no significant correlation to cervical cancer mortality rates. Conclusion This study found a temporal stabilization and spatial independence trend of cervical cancer mortality rates in women from Natal, as well as the absence of correlation between these rates and the average family income of the of the participating women distributed by neighborhoods. In view of this, changes in the public policies should be made aimed at preventing the disease; adopting these measures could positively impact the screening program, improving the coverage of Pap smears and immunization campaigns against HPV, in order to reverse this trend and achieve a reduction of mortality rates.(AU)


RESUMEN Objetivo Describir las tasas de mortalidad por cáncer de cuello uterino y sus tendencias, así como analizar las correlaciones espaciales de este tipo de cáncer en Natal-RN, Brasil, entre 2000 y 2012. Materiales y Métodos Para el análisis estadístico se utilizaron el modelo de regresión lineal simple, la estimación empírica de Bayes y el índice Moran Global. Resultados La tasa de mortalidad por cáncer de cuello uterino en Natal, estandarizado por rango de edad, fue 5.5 por cada 100 000 mujeres. Todas las series históricas para los coeficientes estudiados se clasificaron como estables. El índice Moran Global obtenido fue 0.048, con un valor p de 0.300 para la correlación de prueba espacial entre vecindarios. El ingreso familiar promedio por vecindario no mostró correlación significativa con las tasas de mortalidad por cáncer de cuello uterino. Conclusión En este estudio se observó una tendencia temporal de estabilización e independencia espacial de la tasa de mortalidad por cáncer cervical en mujeres de Natal, así como la ausencia de correlación entre estas tasas y el ingreso familiar promedio de las mujeres participantes, distribuidas por vecindarios de la ciudad. En vista de esto, se sugiere que se adopten cambios en las políticas públicas dirigidas a la prevención de la enfermedad que apunten a medidas que puedan tener un impacto positivo en el programa de monitoreo, mejorando la cobertura de la prueba de Papanicolaou, así como de las campañas de vacunación contra el VPH, con el objetivo de revertir esta tendencia y lograr una reducción en las tasas de mortalidad de la enfermedad.(AU)

BACKGROUND The genus Flavivirus includes a variety of medically important viruses, including dengue virus (DENV) and Zika virus (ZIKV), which are most prevalent in Brazil. Because the clinical profile of patients affected by different DENV serotypes or ZIKV may be similar, the development of new methods that facilitate a rapid and accurate diagnosis is crucial. OBJECTIVES The current study aimed to develop an improved reverse transcription-polymerase chain reaction (RT-PCR) protocol for universal detection of flaviviruses by using semi-nested primers that discriminate between DENV serotypes and ZIKV. METHODS The bioinformatics workflow adopted for primer design included: (1) alignment of 1,442 flavivirus genome sequences, (2) characterisation of 27 conserved regions, (3) generation of a primer set comprising 77 universal primers, and (4) selection of primer pairs with greatest coverage and specificity. Following primer design, the reaction was validated in vitro. The same approach was applied to the design of primers specific for DENV and ZIKV, using a species-specific sequence database. FINDINGS The new assay amplified an 800-806 nt variable region of the NS5 gene and allowed discrimination of virtually all flavivirus species using reference-sequence comparison. The 800-806 nt fragment was validated as a template for a semi-nested multiplex PCR using five additional primers for the detection of DENV and ZIKV. These primers were designed to generate amplicons of different sizes, allowing differentiation of the four serotypes of DENV, and ZIKV using agarose gel electrophoresis. MAIN CONCLUSIONS The bioinformatics pipeline allowed efficient primer design, making it possible to identify the best targets within the coding region of the NS5 protein. The multiplex system proved effective in differentiation of DENV1-4 and ZIKV on a 2% agarose gel. The possibility of discriminating DENV serotypes and ZIKV in the same reaction provided a faster result consuming less sample. Introduction: Due the growing aging population, namely in Portugal, there are few programs to enhance physical, emotional, and psychological well-being. Aim: To determine the perception of wellbeing, vitality and body awareness in a group of older women. Method: Descriptive qualitative study carried out during a psycho-corporal intervention in group of older women. The intervention consisted on the implementation of a program "Vivo no Corpo", throughout 12 weeks in weekly sessions of 90 minutes at a unit health in the city of Porto. Data for qualitative analysis were collected from the diaries of the participants in the program and were written during the sessions. The sample corresponds to women group, (n= 10) with a mean age of 68.4 years. Results: The participants perceived well-being throughout the program, as well as increased body awareness, which facilitated the cohesión of women throughout the sessions. Conclusions: Applying systematically bioenergetic analysis in group can be a tool and technique of intervention that promotes well-being in older women. Qualitative data were collected from session analysis of diaries. Results highlighted enhance of well-being perception throughout sessions, higher body perception and group cohesión.


Introducción: Con el creciente envejecimiento poblacional mundial, en Portugal, específicamente, se han diseñado programas de intervención en la población mayor, para aumentar su bienestar físico, emocional y psicológico. Objetivo: Determinar la percepción del bienestar, la vitalidad y la conciencia corporal en un grupo de mujeres mayores. Método: Estudio descriptivo-cualitativo en que se realizó una intervención psicocorporal, en un grupo de mujeres mayores. La intervención consistió en la implementación de un programa Vivo no Corpo, durante 12 semanas, en sesiones semanales de 90 minutos, en una unidad de salud de la ciudad de Oporto. Los datos para el análisis cualitativo se recolectaron a partir de los diarios de las participantes, en el programa y fueron escritos durante las sesiones. La muestra corresponde a mujeres (n = 10) con edad media de 68,4 años. Resultados: Las participantes percibieron aumento del bienestar a lo largo del programa, así como aumento de la comprensión de su cuerpo, lo que facilitó la cohesión de las mujeres a lo largo de las sesiones. Conclusiones: El análisis bioenergético aplicado en grupo, de forma sistemática, puede ser una herramienta y una técnica de intervención que promueve el bienestar en las mujeres mayores.

ABSTRACT Despite its important effect on the maintenance of tritrophic interactions among plants, insect herbivores, and ants, there is still a paucity of natural history and basic biology information involving trophobiosis among Heteroptera stink bugs. Here, based on previous observations of a new trophobiotic interaction between Edessa rufomarginata (De Geer, 1773) and Camponotus rufipes (Fabricius, 1775) ants, we describe the chemical profile of the honeydew obtained by Gas Chromatography–Mass Spectrometry. There were mainly three different sugars (trehalose, glucose, and sorbose) within our samples. The extrafloral nectaries of Caryocar brasiliense Camb., the host plant of E. rufomarginata, attracts a wide assemblage of Cerrado ants with varying aggressiveness toward herbivores. Therefore, this facultative trophobiotic interaction may allow the survival of the stink bug while feeding on the risky, highly ant-visited plant. Given the rarity of trophobiotic interactions between Pentatomidae species and ants and considering a zoological perspective within this family, here we discuss the ecological and evolutionary routes that may allow the rise of these interactions.

ABSTRACT

To characterize the socioeconomic and demographic aspects of sickle cell disease patients from the state of Rio Grande do Norte (RN), Northeast Brazil, and their adherence to the recommended treatment. Methods: This cross-sectional descriptive study was performed at referral centers for the treatment of hematological diseases. One hundred and fifty-five unrelated individuals with sickle cell disease who went to these centers for outpatient visits were analyzed. All the patients, or their caregivers, were informed about the research procedures and objectives, and answered a standardized questionnaire. Results: The patients were predominantly younger than 12 years old, self-declared as mulatto, lived in small towns fairly distant from the referral center, and had low education and socioeconomic levels. Individuals who were ten or younger were diagnosed at an earlier age. Almost 50% of the patients were taking hydroxyurea, 91.4% reported having received pneumococcal/meningococcal vaccinations and 76.1% received penicillin as antibiotic prophylaxis. However, the majority of them reported having difficulties following the recommendations of the physicians, mainly in respect to attaining the prescribed medications and transportation to the referral centers. Conclusion: These individuals have a vulnerable socioeconomic situation that can lead to an aggravation of their general health and thus deserve special attention from the medical and psychosocial perspectives. Thus, it is necessary to improve public policies that provide Brazilian sickle cell disease patients with better access to medical treatment, living conditions, and integration into society.

Introdução: O atendimento odontológico vem sendo estimulado desde o período pré-natal. Contudo, existem crianças na idade pré-escolar que nunca foram conduzidas a uma consulta odontológica. Objetivo: O objetivo desde estudo foi analisar a utilização dos serviços odontológicos por pré-escolares, de um a cinco anos, residentes em Brejo do Cruz-PB, município de pequeno porte pertencente ao sertão paraibano. Material e método: Este estudo caracterizou-se por ser do tipo descritivo, transversal, com abordagem quantitativa dos dados. A amostra constou de 82 crianças, matriculadas em uma creche pública municipal, e representadas por seus pais/responsáveis, aos quais se aplicou um formulário semiestruturado. Resultado: Observou-se que 76,8% das crianças nunca utilizaram os serviços odontológicos. Do total de crianças que procuraram atendimento, 84,2% foram conduzidas aos serviços públicos de saúde bucal. O principal motivo entre aqueles que levaram suas crianças aos serviços de saúde bucal foi "consulta de rotina", correspondendo a 62,9% dos casos. A maioria dos entrevistados (62,2%) acredita que as crianças não necessitam de atendimento odontológico. Observou-se, ainda, que 79,3% dos responsáveis pelas crianças, não encontraram nenhuma dificuldade quanto ao acesso aos serviços odontológicos. Com relação à condição financeira, não se identificaram famílias que vivem com mais de três salários mínimos. Conclusão: A utilização dos serviços odontológicos por pré-escolares mostra-se deficiente na localidade pesquisada. Porém, constatou-se que não há dificuldades em relação ao acesso aos serviços de saúde bucal para o reduzido percentual de responsáveis que se interessaram em conduzir suas crianças ao consultório odontológico. .


Introduction: The dental care has been stimulated from the prenatal period. However, there are children in the preschool age who have never been taken to a dental appointment. Objective: The aim of the study was to analyze the use of dental services for preschool children aged from one to five years living in a small city belonging to the semi-arid of Paraiba. Material and method: This study was descriptive, cross-sectional with quantitative data approach. The sample consisted of 82 children enrolled in a public kindergarten hall, and represented by their parents/guardians, to which was applied a semi-structured form. Result: 76.8% of children never used dental services. Out of all children who sought care, 84.2% were conducted to public oral health. The main reason among those who took their children to oral health services was "routine visit", corresponding to 62.9% of cases. Most respondents (62.2%) believe that children do not need dental care. 79.3% of those responsible for children found no difficulty in access to dental services. With respect to the financial condition, there were no families living with more than three minimum wages. Conclusion: The use of dental services for preschool shows up poor in the locality searched. However, it was found that there are no difficulties regarding the access to oral health services for the small percentage of caregivers who were interested in driving their children to the dentist. .

Background: Despite being recommended as a compulsory part of the school curriculum, the teaching of basic life support (BLS) has yet to be implemented in high schools in most countries. Objectives: To compare prior knowledge and degree of immediate and delayed learning between students of one public and one private high school after these students received BLS training. Methods: Thirty students from each school initially answered a questionnaire on cardiopulmonary resuscitation (CPR) and use of the automated external defibrillator (AED). They then received theoretical-practical BLS training, after which they were given two theory assessments: one immediately after the course and the other six months later. Results: The overall success rates in the prior, immediate, and delayed assessments were significantly different between groups, with better performance shown overall by private school students than by public school students: 42% ± 14% vs. 30.2% ± 12.2%, p = 0.001; 86% ± 7.8% vs. 62.4% ± 19.6%, p < 0.001; and 65% ± 12.4% vs. 45.6% ± 16%, p < 0.001, respectively. The total odds ratio of the questions showed that the private school students performed the best on all three assessments, respectively: 1.66 (CI95% 1.26-2.18), p < 0.001; 3.56 (CI95% 2.57-4.93), p < 0.001; and 2.21 (CI95% 1.69-2.89), p < 0.001. Conclusions: Before training, most students had insufficient knowledge about CPR and AED; after BLS training a significant immediate and delayed improvement in learning was observed in students, especially in private school students. .


Fundamento: Embora recomendado como parte obrigatória do currículo escolar, o ensino de suporte básico de vida (SBV) ainda não foi implementado no ensino médio na maioria dos países. Objetivo: Comparar o conhecimento prévio e o grau de aprendizado imediato e tardio entre alunos do ensino médio de uma escola pública e outra privada, após capacitação em SBV. Métodos: Inicialmente, trinta alunos de cada escola responderam a um questionário sobre ressuscitação cardiopulmonar (RCP) e o uso do desfibrilador externo automático (DEA); em seguida, receberam treinamento teórico-prático de SBV e foram reavaliados imediatamente e seis meses após o curso. Resultados: As taxas de acertos dos alunos de ambas as escolas nas avaliações prévia, imediata e tardia apresentaram diferenças estatisticamente significantes e melhor desempenho da escola privada em relação à escola pública nas três avaliações, respectivamente, 42% ± 14% vs. 30,2% ± 12,2%, p = 0,001; 86% ± 7,8% vs. 62,4% ± 19,6%, p < 0,001 e 65% ± 12,4% vs. 45,6% ± 16%, p < 0,001. A razão de chance do total das questões mostrou um melhor rendimento da escola privada em relação à pública nas três avaliações, respectivamente: 1,66 (IC95% 1,26-2,18), p < 0,001; 3,56 (IC95% 2,57-4,93), p < 0,001 e 2,21(1,69-2,89), p < 0,001. Conclusões: Antes do treinamento, a maioria dos estudantes tinha um conhecimento insuficiente sobre RCP e DEA; após o curso de SBV, observou-se uma melhora significativa tanto imediata como tardia no aprendizado dos estudantes, notadamente para os alunos da escola privada. .

A inflamação é uma estratégia de defesa inata que evoluiu nos organismos superiores, contra agentes invasores externos e moléculas nocivas. Estudos recentes mostram que a inflamação opera como um sistema muito mais complexo do que se imaginava antes, em termos moleculares, envolvendo diversos processos na sua iniciação, regulação e resolução. Na última década, tornou-se evidente que a inflamação crônica está fortemente associada com câncer, desempenhando um papel importante na tumorigênese. Uma das causas de inflamação crônica são as infecções persistentes por agentes patogênicos virais, com destaque para o papilomavírus humano (HPV). A ligação entre a inflamação crônica decorrente da infecção persistente por agentes infecciosos e o câncer está atualmente sendo apontada como um dos mecanismos-chave para o controle do surgimento de novos casos de carcinomas. Por esse motivo, consultamos os bancos de dados eletrônicos PubMed/Medline, Lilacs, Embase e SciELO, sem restrição linguística, à procura de artigos que abordassem avanços recentes sobre os mecanismos envolvidos na inflamação, sua participação no processo de carcinogênese, com ênfase na correlação entre inflamação e o desenvolvimento do câncer associado ao HPV, os quais foram incluídos na presente revisão.(AU)


Inflammation is an innate defense strategy that evolved in higher organisms against external invaders and harmful molecules. Recent studies show that inflammation operates as a much more complex system than imagined before, in molecular terms, involving many processes in its initiation, regulation and resolution. In the very last decade, it has become evident that chronic inflammation is strongly associated with cancer and plays an important role in tumorigenesis. One of the causes of chronic inflammation are persistent infections by viral pathogens, particularly human papillomavirus (HPV). The link between chronic inflammation resulting from persistent infection by infectious agents and cancer is currently being suggested as a key mechanism for controlling the appearance of new cases of carcinoma. Therefore, we consult electronic databases such as PubMed/Medline, Lilacs, Embase and SciELO, without language restriction, looking for papers discussing recent advances on the mechanisms involved in inflammation, their participation in the process of carcinogenesis, with emphasis on the correlation between inflammation and the development of cancer associated with HPV, which were included in this review.(AU)

ABSTRACT

Este trabalho teve como objetivos avaliar a germinação in vitro e a aclimatação de plantas de sucupira-preta (Bowdichia virgilioides). Sementes escarificadas e não escarificadas foram inoculadas em tubos de ensaio contendo meio de cultura MS ou WPM, com 100 ou 50% dos sais e vitaminas; suplementados com dois tipos de aditivos, carvão ativado e PVP, e mantidas em sala de cultura. Utilizou-se um delineamento inteiramente casualisado em esquema fatorial 2x4 (dois aditivos e quatro formulações de meio de cultura) com quatro repetições e seis sementes por repetição. Aos 30 dias obteve-se o percentual de germinação e o índice de velocidade de germinação (IVG). As plantas foram transferidas para tubetes, contendo substrato e aclimatadas em casa de vegetação, onde permaneceram durante 60 dias, sendo transferidas para casa de sombra, onde permaneceram por mais 60 dias. Aos 30, 60, 90 e 120 dias avaliaram-se o percentual de sobrevivência e a altura das mudas aclimatadas. A germinação in vitro de sementes escarificadas apresenta os maiores percentuais utilizando-se os meios de cultura MS e WPM reduzidos à metade da concentração dos sais e vitaminas. A germinação ocorre independentemente do aditivo utilizado tanto para sementes escarificadas como para as não escarificadas. A aclimatação das plantas germinadas in vitro ocorre independentemente do histórico de aditivos ou meios de cultura utilizados.


Thiss tudy objectived to evaluate the in vitro germination and acclimatization of plants sucupirapreta (Bowdichia virgilioides). Scarified and not scarified seeds were inoculated in test tubes containing culture medium MSorWPM, 100 or 50% of salts and vitamins, supplemented with two types of additives, activated charcoal and PVP, and kept in culture room. We used acompletely randomized in a 2x4 factorial design (2additives and 4culture medium formulations) with 4 replications and6 seeds per replicate. At 30 days gave the germination percentage and germination speed index(IVG). Plants were transferred to tubes containing substrate and acclimatized in a greenhouse, where they remained for 60days and transferred to a dusk house, where they remained for another 60days.At 30, 60, 90 and 120 days, we assessed the percentage of survival and seedling height acclimated. The in vitro germination of scarified seeds had the highest percentage using culture media MS and WPM halved the concentration of salts and vitamins. Germination occurred regardless of additive used both to scarified and note scarified seeds. The acclimation of plants germinated in vitro occurred regardless of additives history or culture media used.

CONTEXT AND OBJECTIVE: The Papanicolaou (Pap) test has been shown to be effective in preventing cervical cancer. However, both the national and international literature shows that Pap testing has not reached the level of coverage desired. The objective of this study was to assess women's knowledge, attitudes and practices regarding the Pap test and to investigate whether there are any associations between these three factors and the women's sociodemographic characteristics. DESIGN AND SETTING: Cross-sectional descriptive study conducted in Floriano, Piauí. METHODS : The study was conducted among 493 women between November 2009 and December 2010. A questionnaire with precoded questions was sed, and the responses were analyzed in terms of appropriateness in relation to the Pap test. RESULTS : The degrees of adequacy of knowledge, attitudes and practices regarding the Pap test were 36.7%, 67.2% and 69.6%, respectively. Among the main barriers against testing, absence of symptoms and a sense of embarrassment were the most notable. CONCLUSIONS: Women who visit doctors periodically had the most appropriate practices regarding the Pap test, but their knowledge of the procedure was poor. This suggests that these women were not receiving adequate information about the benefits of periodic testing. .


CONTEXTO E OBJETIVO: O exame de Papanicolaou já mostrou efetividade na prevenção de câncer do colo do útero. A literatura nacional e internacional tem mostrado que o exame de Papanicolaou não tem alcançado o índice de cobertura desejado. O objetivo deste estudo foi avaliar os conhecimentos, atitudes e práticas entre mulheres em relação ao exame de Papanicolaou e verificar se existe associação entre esses comportamentos e as características sociodemográficas. TIPO DE ESTUDO E LOCAL: Estudo descritivo e transversal realizado em Floriano, Piauí. MÉTODOS: O estudo foi conduzido com 493 mulheres no período de novembro de 2009 a dezembro de 2010. Utilizou-se m questionário com perguntas precodificadas, cujas respostas foram analisadas quanto à adequação dos comportamentos em relação ao exame. RESULTADOS: Os graus de adequação dos conhecimentos, atitudes e práticas em relação ao exame foram de 36,7%, 67,2% e 69,6%, respectivamente. Dentre as principais barreiras para a sua realização, destacaram-se a ausência de sintomas e a vergonha. CONCLUSÃO: As mulheres que realizam consultas periodicamente apresentam prática mais adequada, porém com baixa adequação de conhecimentos frente ao procedimento, sugerindo que não estejam recebendo as informações adequadas sobre os benefícios da realização periódica do exame de Papanicolaou. .

ABSTRACT

Os implantes osseointegrados promoveram importantes mudanças nas formas de reabilitação de pacientes parcial ou totalmente desdentados. Os pacientes totalmente desdentados apresentam clara predileção pelas próteses fixas implantossuportas, sobretudo porque proporcionam maior eficiência mastigatória e conforto, menos reparo e manutenção, além de favorecer o aspecto psicológico, uma vez que elimina o caráter removível das overdentures. Dentre os possíveis desenhos de próteses fixas as do tipo protocolo são as que apresentam maior aplicação clínica, e se caracterizam na mandíbula pela colocação de 4 a 6 implantes na região anterior, entre os forames mentuais, e cantilever distal bilateral para substituir os dentes posteriores. Na maxila recomenda-se a colocação de 6 a 8 implantes. Neste tipo de prótese utiliza-se uma infraestrutura metálica e uma base de resina para uni-la aos dentes de resina acrílica. O objetivo deste trabalho foi relatar um caso clínico de próteses totais fixas tipo protocolo bimaxilares, em que foram adotados critérios bem definidos de diagnóstico e planejamento, e empregadas técnicas cirúrgicas e protéticas com fundamentação científica.


The dental implants promoted important changes in the forms of rehabilitation of partially or fully edentulous patients. Edentulous patients show clear preference for implant-supported fixed prosthesis, mainly because they provide greater chewing efficiency and comfort, less repair and maintenance, in addition to promoting the psychological aspect, considering that eliminates the character removable of overdentures. Among the possible designs of the fixed prosthesis, the type protocols are the which have greater clinical application, and characterized by placing the jaw 4-6 implant in the anterior region between the foramina mentuals, and bilateral distal cantilevers to replace the posterior teeth. In the maxilla is recommended to put 6-8 implants. In this type of prosthesis is used a metal infrastructure and a resin base to attach it to the teeth of acrylic resin. The objective of this study was to report a case of protocol type full-arch fixed prosthesis bimaxillaries with employment of well-defined criteria for diagnosis and planning, and surgical and prosthetic techniques scientific evidence-based.

Understanding the impact of Asian soybean rust on soybean yield is of great importance in the crop simulation model for this crop become it is possible to predict yield using different sowing dates and growth conditions. The goal of this study were to evaluate the performance of two soybean cultivars in Triângulo Mineiro/Alto Paranaíba, MG, Brazil and the effects of soybean rust on the yield of these cultivars using the CSM-CROPGRO Soybean model. Two soybean cultivars "NK 7074" (early) and "UFUS-Impacta" (medium late), which differ in their development cycles, were growing in Uberaba city during the 2009/2010 growing season. The validation for cultivar "UFUS-Impacta" was conducted comparing the measured and simulated yield data considering three different sowing dates in the "Uberlândia" city during the 2002/2003 growing season. Daily meteorological data obtained from six meteorological stations of the National Institute of Meteorology (INMET). To determine the performance of the soybean cultivars and the effect of soybean rust on yield, three different scenarios were used: no occurrence of rust (NOR) and occurrence of rust with inoculum concentrations of U5.000 and U10.000 urediniospores/mL. For all environments studied, the early cultivar had the best performance than the medium late cultivar. Soybean rust had the most effect on yield for the U10.000 scenario than for the U5.000 scenario. The best soybean performance occurred for "Araxá" and "Uberaba" cities. The SouthSoutheast area of the "Triângulo Mineiro/Alto Paranaíba" region was the most sensitive to the effect of rust on yield compared to the North region.


Compreender o impacto da ferrugem asiática na produtividade da soja é de grande importância para os modelos de simulação dessa cultura, pois pode-se prever a produtividade utilizando-se diferentes datas de semeadura e condições de crescimento. O objetivo deste estudo foi analisar a performance de duas cultivares de soja na região do Triângulo Mineiro/Alto Paranaíba, MG, Brasil e os efeitos da ferrugem asiática da soja na produtividade desses cultivares utilizando o modelo CSM-CROPGRO Soybean. Duas cultivares de soja NK 7074 (precoce) e UFUS-Impacta (semitardia), as quais diferem nos seus ciclos de desenvolvimento, foram cultivadas em Uberaba na safra 2009/2010. A validação para a cultivar UFUS-Impacta foi conduzida comparando os dados observados e simulados de produtividade considerando três diferentes datas de semeadura na safra 2002/2003 em Uberlândia, MG. Foram utilizados dados meteorológicos diários de seis estações meteorológicas do Instituto Nacional de Meteorologia (INMET). Para determinar o desempenho das cultivares de soja e o efeito da ferrugem na produtividade, utilizou-se três diferentes cenários denominados de: não ocorrência de ferrugem (NOR) e ocorrência de ferrugem nas concentrações de inóculo de U5.000 e U10.000 urediniósporos/mL. Para todos os ambientes estudados, a cultivar precoce teve o melhor desempenho em relação a cultivar semi-tardia. A ferrugem da soja teve maior impacto na produção para o cenário U10.000 do que para o cenário U5.000. O melhor desempenho das cultivares de soja foram para as cidades de Araxá e Uberaba. A área Sul-Sudeste do Triângulo Mineiro/Alto Paranaíba foi a mais sensível ao efeito da ferrugem na produtividade em comparação com a região norte.

Introdução: crianças infectadas pelo vírus HIV por transmissão vertical geralmente apresentam baixopeso. Objetivo: estudar as características antropométricas e sociodemográficas de crianças soropositivaspara o HIV, infectadas por transmissão vertical, que faziam uso de antirretrovirais. Método: estudotransversal, descritivo com 30 crianças soropositivas, em uso de terapia antirretroviral (TARV). Aavaliação nutricional foi feita, segundo os índices de altura para idade, peso para idade e Índice de MassaCorporal (IMC) para idade. Outros dados foram obtidos por meio de um formulário semi-estruturado,respondido pelos pais/responsáveis pela criança. Resultados: a maioria das crianças era meninos (60%).As mães tinham até 8 anos de estudo (73,4%), viviam com até 1 salário mínimo e somente 3,3% usaramTARV durante a gestação e parto. A avaliação do estado nutricional das crianças apontou percentual de13,3% para baixo peso para idade e 23,3% de baixa estatura para idade. Em todas as idades foramobservados déficits estaturais, embora os mesmos não se apresentassem de forma regular e crescente como aumento da idade. Em relação ao IMCI observou-se adequação de peso para altura e sobrepeso.Conclusão: a desnutrição crônica traduzida pela baixa estatura foi comum nas crianças HIV positivasestudadas, agravada, possivelmente, pela situação socioeconômica materna e a baixa adesão ao prénatal.


Introduction: vertical transmission HIV virus infected children generally present a low weight.Objective: to study the socio-demographic and anthropometric characteristics of HIV positive childreninfected by vertical transmission, which made use of antiretrovirals. Methods: transversal study,descriptive with 30 HIV serum-positive children, under the use of antiretroviral therapy (ART). Thenutritional evaluation has been performed according to the height-for-age, weight-for-age indexes andBody Mass Index (BMI) for-age. Other data have been collected through a semi-structured form,answered by the parents who are responsible for the child. Results: most of the children were male (60%).The mothers had a maximum of 8 years of formal education (73.4%) and live with up to one minimumwage and only 3.3% used ART during pregnancy and delivery. The evaluation of the children?s nutritionalconditions has pointed out a percentage of 13.3% for low weight to the age and 23.3% for low height tothe age. In all ages it was observed structure deficits, although they did not show up in a regular andraising basis, mainly with aging. As for BMI, it was observed an appropriation of weight to height andoverweight. Conclusion: Chronic malnutrition translated by low height was common amongst the studiedHIV positive children, possibly aggravated by the social-economical situation of the mother and the lowadhesion to prenatal programs.

ABSTRACT

A neurocisticercose é uma doença caracterizada pelo envolvimento do sistema nervoso central pelo estágio larval intermediário do parasita Taenia solium. O processo de degeneração da larva e a reação inflamatória do organismo causam os sintomas clínicos. Relatamos a reativação clínica e radiológica de uma forma nodular calcificada e assintomática há mais de 20 anos. O tratamento antiparasitário mostrou boa resposta.


Neurocysticercosis is a disease characterized by the involvement of the central nervous system by the intermediate larval stage of the parasite Taenia solium. The larva degeneration process and the inflammatory reaction of the body cause clinical symptoms. The authors report a case of clinical and radiological reactivation of nodular calcified neurocysticercosis in a patient who was asymptomatic for more than 20 years. Antiparasitic treatment showed a good response.

ABSTRACT

A cerebelite aguda é uma síndrome inflamatória rara frequentemente caracterizada por rápida disfunção cerebelar. Neste estudo relatamos os achados de imagem do caso de uma criança com cerebelite aguda, herniação tonsilar e hidrocefalia hipertensiva. O agente etiológico não foi descoberto. O tratamento foi conservador, com manitol e corticoide. A análise evolutiva por imagem demonstrou resolução do quadro clínico sem sequelas.


Acute cerebellitis is a rare inflammatory syndrome frequently characterized by fast onset of cerebellar dysfunction. The present case report describes imaging findings in a child with acute cerebellitis, tonsillar herniation and hypertensive hydrocephalus. The etiologic agent has not been determined. A conservative management was adopted, with corticoid and diuretic drugs. Imaging follow-up demonstrated resolution of the clinical condition with no sequela.
